Faculty/Staff One Card Authorization Form
The One Card is the only official form of identification at the North Carolina School of the Arts. Your OneCard acts as your school ID, access card, meal plan card and debit card for campus purchases & services It should be carried at all times and must be presented for access to campus facilities, services, campus sponsored events, and more.
All new students obtain their card during the registration process or at all other times in the OneCard Office. Replacement cards cost $10 and are obtained in the OneCard Office.

With security readers on most doors, we provide a level of convenience and security that could not be provided with a conventional keyed system. Your card carries the details of your meal plan and has a debit account feature that makes purchases on campus convenient, fast and easy. (Cash cannot be drawn on the account)

You can add money to your OneCard at the following locations:
Semans Library Value Transfer Station located in the lobby adjacent to the circulation desk
The VTS machine takes cash only ($1, $5, $10, or $20 bills)

Checks should be made payable to NCSA One Card Office.
Lost or found cards must be reported immediately to the OneCard Office during regular business hours, or NCSA Police after hours and weekends. Temporary cards can be issued at no charge for a limited amount of time until the card is found or a new one is made. Temporary cards are available from NCSA Police after 5 pm Monday – Friday and on the weekends. The cost to replace a temporary card is $20.
The One Card is a multipurpose card. It provides a campus ID, access, meal plan, and the ability to make a variety of purchases on campus with the debit feature.
The One Card Office is located on the lower level of the Hanes Student Commons building adjacent to the Computer Lab and the Kenan Student Life Center. You can contact the One Card Manager at (336) 770-1429. After hours, any One Card problems should be directed to the NCSA Police office located on the ground floor under the Drama Gym on the Service Road. Their phone number is (336) 770-3321. Their emergency number is 55.
The One Card is the property of NCSA and must be returned to School officials upon request. The card must be carried by users at all times. It is not transferable. Unauthorized use, tampering, or alteration may result in disciplinary action and loss of privileges. Lost or found cards should be reported to NCSA Police.
Users should note the following features of the One Card:
- Your One Card is your campus identification. All students, faculty, and staff are required to have their One Card with them while they are on campus.
- If you have a meal plan, you are required to have your One Card with you to receive a meal. You need to give your card to the attendant to swipe through the register.
- Your One Card is required to access various buildings and rooms on campus. Various plans may be assigned to your card to allow or restrict your access to areas on campus, as needed.
- The One Card is required in the Library and Wellness Center.
- Your One Card is unique to you. It is not transferable and only for your personal use. Allowing someone else to use your card or using someone else’s card is prohibited and may result in suspension of One Card privileges and/or other disciplinary actions.
- Your One Card has a debit feature that allows you to make a variety of purchases on campus. It can be used at the Campus Store, the Pickle Jar (snack bar), Health Services, Mail Center, vending machines, and laundry machines. It can be used to pay for various trips and functions sponsored by Student Activities. It is required to make copies in the Library.
- Since your One Card is a debit card, it must maintain a balance, if you wish to make purchases.
- The money you put on your card must be used to make purchases on campus.
- You cannot withdraw cash from your card.
- As long as you are a student, the balance on your One Card Debit plan will carry over from term to term and year to year.
- Upon graduation or withdrawal from NCSA, any debit account with a balance of $25 or higher will be eligible for a refund provided you complete a One Card Refund Request form. You must apply in person with the One Card Office for a refund at the end of term in which you graduate or at any other time during the term if you officially separate from NCSA. Refunds will not be received until all school fees are paid in full. All refund checks will be mailed to the payee. Refunds take approximately 2 months for processing.
- Any balance not used by the end of the summer session is forfeited.
- You are responsible for paying for all purchases regardless of whether or not the One Card allows it.
- If you lose your One Card, you should immediately contact the One Card Office (NCSA Police when the One Card Office is closed.) If you have cash on your card and use it for meals or a debit card, it could be used by anyone until it is disabled. The fee for replacing a lost card is $10.00. There is no charge for replacing a damaged card (unless beyond normal wear and tear), if you bring it with you to the One Card Office. Everyone needs have to have a new picture taken and card printed every three years. The fee for replacing a lost temporary card is $20.00. You may pay for the lost card with cash, check or debit account on the One Card.
- You can add money to your One Card at the One Card Office or the Value Transfer Station (VTS) in the library by inserting $1, $5, $10, or $20 bills. A fee of $25 is charged for all returned checks.
- You can check your balance at any reader (Campus Store, Dining Hall, Pickle Jar, Student Activities, The What, Fitness Center, Mail Center) when a purchase is made or at the VTS in the library. You can also come by the One Card Office to check your balance.
In the event of a campus emergency or national alert, the administration reserves the right to REQUIRE all students, faculty and staff to wear their identification cards (One Card) clearly visible on their persons.
