Transcripts must be requested in writing and must include your signature. The School of the Arts cannot accept transcript requests by email since, by law, all transcript requests must bear your signature. You can also download our Summer Program Transcript Request Form or you may pick up a transcript request form for the Summer Program at the Office of Admissions or you may make your request by letter. Please include the following information:

• Your name, and any other name you used at NCSA
• Your dates of attendance
• Your Social Security Number
• Please indicate whether you were here in High School or College
• A complete address(es) to which you would like the transcript(s) to be sent
• Your return address, and telephone number
• Your signature
• Payment ($5.00 per transcript)
• E-mail address if possible

Your request will be returned to you if the above information is incomplete.

The charge for all other transcripts will be $5.00 per transcript.

Please allow five to seven business days after receipt of your request for your transcript to be sent. During peak times of the year it may take longer.

Please note: We are unable to process transcript requests on the day that they are received.
